I wore them exactly as pictured . After a couple of days, I figured out how to "unhook" one strap so I could stand up while peeing. It was awkward at first but got used to it. Just don't wait until you absolutely have to go so you are not fumbling around with the straps, etc. I was able to work all day and walk the dog for almost an hour after getting home wearing this. Now I've had my surgery so I should have no further need but definitely recommend as velcro is still holding up. Before I got this belt, I was miserable. I have an internal inguinal hernia. I have no bulge noticeable on my lower ab or groin. I couldnt walk 100 yards without stopping and getting nauseous and almost vomiting. The only relief icould get from the pain was lying flat on my back. Anytime I would stand or walk, I was in incredible pain. I started wearing this belt and almost immediately started to feel better and was able to get back to work and even take my dog for walks again. Im not able to have surgery for 3 weeks, but with this belt, I am confident that I will be able to make it that long.
