Q: How do you adhere these to a wall?

The top of the white outer planter has a hole in it. You could use a screw hook, screw, or a nail.

Q: If i have a 4" plant, would I repot it with soil in the insert? Or could I just pop the 4" plant in it's growers pot in the inset?

I think if you’re looking to use the self watering wick, repotting the plant in the sleeve/insert is best so that the wick can reach the soil for watering. I personally don’t use this method and am just using these pots for for hanging my plants so I have my 4” pots simply inserted in the black sleeve/insert instead.
