Q: Does this use 18 v batteries?

Yes. I've only used it on one project so far. Charge doesn't last long, so make sure you have 2 batteries. Does a great job.

Q: Is there an option to plug in with an extension ?

There is no extension 'cord' plug for the belt sander. I have never seen Ryobi offer a corded battery adapter either For the p450 Belt sander I could run it continuously cycling through three 4ah batteries letting the sander do the work and not pressing down on it.
