Hi,S is 8.66 inches,M is 8.46" ,L is 9.44" ,XL is 9.84inches.And they are elastic and can be stretched.

Q: How do you figure out how to determine what size is needed for an individual?

Sorry I can’t be more specific but just took into consideration weight and leg size. For a 257 pound, 6ft man, a large worked fine.

Q: Should the size determination measurements be made at the highest point (thigh or calf) it will be worn or at the midway point ?

My husband wears his mid-thigh. He says they are very comfortable, barely notices it's there. Honestly, we did not even measure.
