Q: Does this come with all of the rolling attachments?

No mine did not come with all of the attachments but I did not order the one that came with them. I have used the one with only the black roller that my doctor had and it works well for whar I needed.

Q: does this come with strap for holding my thick?

It might be awkward, but I think it would work.

Q: How tight does the top need to be gripped to hold together? Gripping is painful for me so I don't know if the roleo is better for me right now.

Hello Michel - Thanks for your question! You should not need to grip the two arms together too tightly in order to effectively apply the trigger point therapy. If gripping things at all is painful for you, consider using the rubber band that comes with the Armaid to hold the handles in place, rather then your hand.
