It is left hand outside the belt or right hand inside the pants

Not sure. I bought it for my S&W M&P Shield. It's a perfect fit for that.

I realized after I ordered it it says 3.1 I have the 4’ is this going to be a problem ?

Will my m&p 9 shield plus 9mm fits the holster?

My Shield 9 Plus without any additional lights or optics fits perfectly, right out of the package, without any adjustments. The slide fits evenly with raised section of the holster, so it doesn't press into your side when wearing it. Nice looking holster and a great Value for a quality product. All I need to find is a holster for the Shield Plus that will accept the Olight Baldr Mini. I would like it to be a Tactical Scorpion.
