Why does it state 0.18 when it starts at 0.39 according to video and pictures?

I'm not sure I don't even look at the size. I just eye ball size it up to the size of cable or wire and strap it. It works for SJO 12 gauge cable up to 2/3 buss drop cable. 10 gauge up to 800 kcmill. I use it very often haven't had to sharpen it or anything. My electrical bag is not hole with out it.

I tried the jokari version of this. The adjustable blade would not stay square to the cable jacket and kept spinning. Does this do the same thing?

I love this tool, not anyone can use it because the design is perfect for its use. I think that you have to learn the tool. It will cut around the cable in one direction, and spin the blade to cut insulation straight down the middle.

Can the tool be adjusted to where it cuts at 90 degrees eliminating a spiral cut?

Yes. The lever turns the cutting blade 90 degrees.
