Can I use this inside an out-of-use refrigerator being stored with the door closed?

I believe you can. I use this on my boat for over winter storage. Usually under the seats which are ventilated, but I also put in cabinets that are not ventilated with the doors closed. In the Spring the containers have water in them.

Is it easy to tell when it needs to be replaced?

Yes, you can tell. It all depends on how much moisture is in the room you set it up in. The directions say to wait until it is all dissolved and full on water in the bottom tub. I just kept an eye on it every few weeks.
