Learned so much from this
This is a great book. I recommend it for all prescribers and nurses who work in pain, palliative care, or hosoice setting. I am almost ashamed about how much I learned considering Inhave been in the field for several years. Written in a clear, understandable, and at times humorous way. I read this book while I was preparing for NP exam for Hospice and Palliatve Care certifiation- on which there were absolutely no drug conversion questions. But I am so glad I read this and it immediately impacted my practice.

Useful for any physician who needs to treat pain with opioids
This book is thoughtfully written, and is an easy read. The author emphasizes the use of intelligent clinical reasoning. When you have decided that opiates are appropriate, using the principles in this book can give you confidence that your regimen makes sense and is safe. I use this book in teaching rotating anesthesiology residents in my pain clinic.

This is the gold standard
This book is not only the gold standard, it is also entertaining. About a year ago I agreed to particpate as the hospice/palliative pain managment pharmacist primarily due to the existence of this book. We use it to guide our therapy and conversions and as pharmacists we universally acknowledge it's importance, necessity and thoroughness. I have almost worn out the copy I purchased a year ago. Do not hessitate, purchase this book. I recommend it all the time. Thank you Dr. McPherson for writing this!

Excellent reivew
I work at the acute care hospital, and many physicians do not feel comfortable with main management. Since we do not have a pain management specialist, they ask for assistant from pharmacist with opioid use, conversion among them and how to manage overall pain control. Since I studied this book, I have better understanding how to handle opioid. I am not a pain management experts, but I know this book can help a lot of pharmacists who handles pain medication daily.

Useful but lacks precision
This book is a useful overview of complex subject. I am dissappointed, however, in the author's imprecise definition of terms. For example, the term potency is clearly defined in pharmacology as determined by the affinity of a drug for the receptor to which it binds and as such, is a property intrinsic to that particular drug. The author correctly states that potency is a function of affinity but INCORRECTLY states that potency is also a function of the drug's "access to the opioid receptor." How much drug ends up initially presented to the target receptor ("access") is determined primarily by the pharmacokinetics of the drug, time from administration and dose administered and is NOT a function of potency. Based on the incorrect statement that potency is partly a function of access to the receptor, the author goes on to state that pharmacokinetic and physicochemical variables account for "differing potency among opioid agonists". The argument can perhaps be made that there there is a practical, functional relationship between receptor access and "potency" but the author fails to make the distinction between her definition and the pharmacologic definition of potency.In as much as definition of terms provides part of the foundation upon which a textbook is built, this kind of imprecision undermines my faith in the book as a whole. The book, however, does contain a great deal of useful, practical information.

This book was recommended before starting my training. Don't have a clue about opoids at all? Or just want to brush up on your skills? This book will become your essential survival tool. Easy to read and understand. It also has many practice questions to build up confidence.

Great book for pharmacists/students to learn the basics of the complex art of opioid dosing. I must say though, I was somewhat surprised at the number of grammatical and typographical errors throughout the book.
