If you look at the second picture, you'll see the ribs in the handle that grip the rope. Simply pull the rope to the desired length and cinch onto the handle. It's very simple with no need to tie knots.

This is a very simple but effective tool for rehab. In fact, it works better than the one I use at the physical therapist. It is merely a small pulley with a rope and 2 handles. The strength of the good arm exercises the arm that needs strengthening both by lifting and allowing the injured arm to lower. Works great for physical therapy for post rotator surgery. The unit fits well and snugly over a door without doing any damage. It is padded nicely.

Yes, the pulley can swivel completely around. It can be used the way you describe.

Q: Are the ropes long enough to use while seated. A need hip surgery and at PT they have me use this while sitting.

Yes it would be long enough to use while seated. I had to put knots in it to shorten it some for my exercises since I was standing & using it to strengthen my shoulder. PT said I could use while sitting for my shoulder exercises also.
