the waist is 40 at least, maybe larger. they are designed as a one size fits all. they have a draw string to pull it tight. I am a waist 34 and they fit great with the draw string pulled.

I'd say that the large size pairs that I own would accommodate 34-47" waists

No they are not lined and going commando would probably end up with some indecent exposure. Comfy otherwise though, but I always wear some sort of underwear with these.
