Q: Would this hold the weight of my camera and a heavy lens for an overhead shot and it mounted by this flap from above, behind the screen?

That would depend on how you plan on mounting it. On the vertical flap where the v mount attaches, there are 1/4" threaded holes.

Q: how to I connect it to Tilta cage?

Dear Friend, Thanks for inquiry. If your cage is equipped with an Arca-Swiss plate, you can directly install the cage on this plate. If your cage is not equipped an Arca-Swiss plate, you can use the small quick release plate in the package to install it at the bottom of the cage and then install it on this plate. Q: Do I need a camera cage for this to screw on or can I just attach it to the camera body itself?

You can screw it directly onto the camera but I would highly recommend getting a cage with arca-swiss on the bottom.
