Q: Does this have to be plugged in to use?

Yep, no on and off switch on remote or chair..just plug and unplug

Q: how do I program the remote to work the chair?

am having problem with the remote control I press a bottom for back all the arrows are on

Q: How tall is the back, from seat base to top?

It is a high back that is a little over 36" from top to bottom of seat.

Q: Two questions: 1) Are there heating elements in both the back and seat? 2) Can the heat be used by itself, without any vibration or massage?

(1) There is only a heating element in the seat. You can barely feel it through your clothing though. You know it's working if you touch it with your hand. (2) The heat cannot be used alone. Just turning the chair on in the first place automatically activates the 1st vibration setting, and then you can activate the heat. But you cannot deactivate the vibration without turning the chair off entirely. Don't buy it for the heating aspect, that is not this chair's strong quality. The chair is comfortable and the vibration is a pleasant perk.
