Q: if i would like to do facials on this bed how many positions does the head rest adjust to?

I use thsi for physical therapy fora knee replacement so I am not sure I can answer your question. I needed the back to to go up and I didn't need the face rest so I haven't used it.

Q: does this table come with a sheet set?

No. There is one that does tho. But it's just as easy to go to Walmart and buy a twin sst. Still a little big but it works

Q: What is the lowest height adjustment?

It is adjustable from complete horizontal (flat) to 90˚. I love my table

Q: does it fit in small spaces

I keep my massage table in the cover and slide it into my closet. I don't know what you mean by "small spaces" but folded up, the table is approx. 36x28x6.
