I have trouble with the thumbs on both hands. Does this require a lot of strength in thumbs?

Yes, I have a great deal of trouble opening it. Most times when I do open it the juice flies and spills everywhere. You need a great deal of thumb and hand strength in order to open this without juice ending up all over the counter! I hate this thing!

does it work for oranges or will they be too big?

This mini citrus juicer works best with lemons and limes. Oranges are probably too big and may create a mess on the counter top.

How do you open it? it says "to open: hold base and rotate top" but maybe i'm trying the wrong way?

Unless they changed it in some way?? Its just a friction fit. Just pull the top off. The problem though is its on there pretty dang tight and forcing a lemon down on it to juice just wedges it on even tighter. It practically guarantees you'll have to pull so hard to open it that the juice will go everywhere except where you want it. The design of this juicer is the real lemon here.

Can one directly juice into a coffee mug?

In regards to your question, sure you can use this directly into a coffee mug. It may not fit perfectly to place on the rims of a coffee mug but you can use over it. Easy to use, simply cut citrus in half, place it onto the pointed reamer, cut-side down, and twist back and forth with a firm pressure. The extracted fruit juice will collect in the reservoir.
