How many screw hole is available on the handle?

If I am looking at the correct handle it’s 4 per handle. Sorry to be not precise but I have fitted several handles of different makes on my doors.

dumb question perhaps but do you get both levers (ie for both sides of a door)? or must I buy 2 items per door

How on earth do you get the metal covers off? They do not move at all!! Is it pull off or unscrew? Very annoying.
