Great little controller for SmartThings!
This is a very solid little four-button fob with a sliding cover. It's about the size of a car key fob, and seems to be solidly built. The buttons and the "cover" are all a brushed silver finish (the Amazon picture looked white to me).I followed the included quick start guide, and got the fob connected to my SmartThings v2 hub easily. It was correctly identified in the automatic device search. I was very happy to learn that it could be configured to trigger automations not only on button press, but also on button hold.My primary use for this was as a remote for my ZWave garage door controller. I was expecting to have to use two buttons for this (one for "open" and one for "close") but I was able to set the fob to open the door on button hold, and close the door on button press. That feature really gives you 8 possible actions from the four buttons.I've configured the other buttons to perform small tasks around my house, and they all work fine. This is extremely handy, and if you have your SmartThings integrated with IFTTT, you can really accomplish a lot.

Extremely difficult setup and unreliable; wait until they build one that's usable
Aeon Labs is really good at packing Z-Wave into small packages. Unfortunately, they are horrible at making those devices either easy to use or reliable. I bought one of this to evaluate for a large project. Just setting it up is a nightmare. There's a little two-pronged "key" in the package which is necessary for setup. Using it correctly is very difficult. If you press too hard, it doesn't work. If you don't press evenly, it doesn't work. If you press too long or too short, it doesn't work. After struggling for over 30 minutes, we finally got it into setup mode. However, it soon lost our setup, and we were back trying to enter setup again. Never did succeed the second time. Tech support basically said to read the manual. (Incidentally, the "manual" is printed in type so small we had to get a magnifier to decode it.) We finally through this gizmo away, Other Aeon Labs devices we have don't seem to retain their settings reliably either. A little less clever and a little more usability and reliability might even make these products actually useful. Otherwise, we suggest you avoid them unless you have incredible patience.

Works well with Vera Plus but it sucks up batteries. I'm replacing one a month. Going to scrap it for a rechargeable one.

Works well once I figured out one quirk
I struggled a little installing this, but got it to work. The smart things driver has both "Aeotec Fob" radio button and "Aeotec Fob Button 1" (...2,3,4) radio buttons on the "Automation" screen. With my SmartThings hub, it did not work properly until I chose the "Aeotec Fob" (no button number) choice, then chose "Button 1" on the next screen. Once I did that it worked fine. The "Fob button 1{2,3,4)" choices did not trigger the automation event. The history showed the button pushes but nothing happened. Now that it's working (hence the "4 stars" rating because of the struggles) I'm carrying it in the car as an "Arm/Disarm" and "Lights On/Lights Off" remote. It's doing what I wanted. Maybe I'll decide to do something with the push-and-hold functions later. I did end up getting a second one for the other car too.

Requires Some Time (and patience) to Get Working With SmartThings
As others have noted, this is a good product. Once it's paired and working and therein lies the problem.The supplied instructions are vague at best and printed in a booklet so small it's virtually impossible to read. Interestingly, if you google for the manual it takes you to the manufacturer's site but the manual linked to has LOTS of missing information. My z-wave network at home consists of a SmartThings v1 hub, an alarm system, cameras, lights, thermostats and is integrated with the Amazon Echo but this device was by far the most difficult thing I have set up so far. I struggled with it for days to get it going and even then it was only after a few e-mails with ST support that things happened.For those that are struggling with it I hope the following will be helpful:Problem - Once paired with ST and configured with the Button Controller SmartApp nothing happened. Button presses did nothing and there was no evidence of anything being recorded in the SmartThings IDE section for that device.My Solution -1. Remove and re-insert the battery2. Reset the fob using the pin to press and hold the "Mode" button on the reverse of the fob for 20 seconds. During this time the LED will start blinking and will blink faster and faster until it eventually stops and shows solid green3. On your phone delete any SmartApps associated with the fob (i.e. Button Controller).4. Login to the SmartThings IDE [...] go to "My Devices" and delete the fob5. Log out of the IDE6. Begin the pairing process again for the fob7. Associate the fob with a room (if you are using groups to groups devices together by location in the house)8. Once it has done pairing use both prongs on the pin to press the "Mode" and "Learn" buttons on the reverse of the fob and hold them in for about 3 seconds. The LED will blink, then turn solid green (THIS IS VERY IMPORTANT AS IT SETS THE FOB INTO A MODE THAT ALLOWS IT TO BE USED AS A REMOTE TO CONTROL DEVICES)9. Go to the SmartApps section in the phone app and install the Button Controller SmartApp. Use it to program the buttons on the fob. Refer to the instruction manual for the order of the buttons so that you get them in the right order. Start out by testing using only one button, maybe to turn a light on/off so that you can see very quickly if it's working.It's important to remove it from your network if you currently have it paired but it's not working. I spent ages trying to finesse it but success only came by removing and re-adding it.Hope this helps someone and saves them some anguish.
