Q: Does this product come with the connecter to mount the camera directly to the clamp without using the extension neck?

If I am interpreting this question correctly, then NO, it does NOT come with the male-CONNECTOR. What mine came with was the neck (which has a male-connector on one end and the female-connector on the other) and a clamp (only the female-connector)--2 pieces total. To connect the camera to the neck OR clamp, you will need to supply your own male-connector unit (that plastic piece that looks like a buckle) to attach to the camera. Once you have that, the camera can connect to the mount in any variation you please (just the clamp, just the neck, or to the neck as its connected to the clamp).

Q: Could this Jaw Clamp also be used to securely mount the Go Pro Hero+ 3 camera onto the Handle bar of a mountain bike?

Not "Securely". You might as well buy the bike mount. This clamp might work OK on smooth roads, anything off road and it would get knocked off.

Q: How would this hold up on motorcycle handlebars behind a wind screen?

There is a GoPro Handlebar Seatpost Mount that might be better. We used that one on a motorcycle ride and it worked great and it wasn't behind a windscreen.
