Q: I need a flapper for toilet Eljer side flush lever?

This kit is for relatively older Eljer toilets with front flush levers (Mine is from the 1970's). If you have a side lever you will need a different kit.

Q: Do you have to remove the tank when replacing the anchor piece the float arm clips into?

Yes you do, so you also need a "tank to bowl" kit. If your existing anchor piece is still good; I'd try to use this kit without replacing the anchor. The anchor in this kit is not as robust as OEM piece and I had some problems with the arms bending in wrd when I tightened things down, so the flapper arm would slip off the pins. If you do get a tank-to-bowl kit I recommend a dab of caulk in the mounting hole in the tank when you insert the new bolts.

Q: What is the measurement between the holes of the fork please? (I want to use the existing anchor)

Q: Does the "U" shaped bracket mount on the left or right bolt as you face the tank?

Assuming that the flush handle on your tank is on the left when you are facing the tank, the U bracket mounts on the right bolt. Be careful installing it. It is much more flexible than the OEM U bracket and tightening the bolt with washers in place caused the sides of the U bracket to pull inward making the arm fit very loosely. I had to do this to stop leaking around the bolt (I also caulked around the bolt shaft). To fix the inward U I had to insert a spacer between the U legs to push them outwards a bit.
