Yes, you get an orifice, cap, and wrap to go with each of the 25 bottles.

Can I boil these to sanitize before filling?

The honest answer is I don't know. The tops of the bottles are plastic, there is a foam seal which inserts into the cap. I did boil the bottles for 10 minutes to sanitize prior to filling but I'm not sure if the caps would be able to tolerate boiling for a longer period of time.

Where are these bottle made?

Well I'm going to say China. The box has Chinese symbols on it
