Q: one review said this chair only stays up or reclines completely flat.(thats it) is this true? the picture shows a 3 position reclining chair.

When I recline the chair, I use the lever on the left to allow motion and then push the lever back to lock the back of the chair in any position from upright to fully reclined. It is NOT limited to any particular intermediate positions.

Q: Does anyone else have this chair constantly fall forward when rolling in? This seems to happen mostly when the footrest isn't 100% of the way back.

I get that problem as well, it's because the gas cylinder isn't under the middle of the seat. The gas cylinder is farther back on the seat so that when it reclines, you don't tip over backwards (that's the physics, not what the other guy was saying). However, the cylinder placement also causes when you try to roll forwards, especially on carpet), there is a much greater torque on the chair forwards, causing the chair to almost tip over.

Q: Are the cushions soft like an overstuffed couch, or more firm/supportive?

Firm and supportive like an office chair. Not soft like a Lazy-boy recliner.

Q: Chair come with only 4 screws, all the same size. Should there be more screws?

The one I got came with 8 screws. 4 of them shorter than the others. The shorter 4 weren't in a bag, they were already inserted in the holes on the chair back. The holes they are supposed to go in when you attach the back to the base. Although you have to remove them in order to slide that part into the base before you use them.
