Item states it's 2% solution but reviewers state they dilute it further? Also shampoos and other ready to apply solutions are 2-4% I'm lost here?

According to studies, solution as week as 0.12% are sufficient for killing microbes in 30 seconds. In fact that is the strength of effective oral rinse. HIbiclens is 4% which is quite strong. I think for washing I would pets I'd probably dilute down to .75 to 1%.

Can you use this to clear dogs tear stains?

Absolutely no. Find out the cause of tear stain in the first place and not trying to clear the aftermath. In my dog’s case, he didn’t have tear stain but had beard stain. Cleared by removing chicken from his diet. And the fur without stain grow so I just trim the stain over time

yes, according to another reply regarding ingredients: usp purified water, alkyl dimethylamine oxide, chlorhexidine gluconate,polyoxyethhylene-6-cocoamide, octylphenoxypolyethoxyethanol, glycerin, polyethylene glycol, hydroxyethylcellulose, fragrance and fdc blue #1.

Is this the same "Chlorhexidine 2% Solution" that I get from my vet to use on my cats' little scratches, irritations, etc.?

Ask your vet. Chlorhexidine may be the active ingredient in what they give you. It's in some soaps, shampoos and wipes. Even this 2% solutiin is highly concentrated and is often dilluted more with plain water for many uses. So it could be this and already dilluted.
