Q: What would happen if these fell out of my ear when I was already 40 feet underwater during a scuba dive? Would I be fine or my head explode?

I may have only snorkeled down to 15 feet or so. I wore them cause I have a ruptured drum from starter pistol going off next to my ear. Swimming in shallow waters I get water running down into my ear canal that hurts. Some other divers told me they don't trust stuff in thier ears because of the pressure. When I went down I could feel the pressure start but remember is had also been about ten years or more since I did any kind of diving so I wasn't used to it. The pressure should hit you but for these to get knocked out of your ears that way would be crazy. You could put allot of pressure on your ear drum but worst case if you're not prepared for that to happen you'll just have to surface and recover. Have you been able to swim without plugs before this would let you know how you'd adjust to pressure. I had no problems with them at all and loved them. Perfect fit and no water into my ears after 8-10 days in and out of the water. I plan on using them again.

Q: Would these ear plugs also work to relieve air pressure while flying?

I would not think so, while ascending the air pressure in your ears is greater than the cabin pressure so you would want your middle ear pressure to equalize which these would probably prevent. While descending the cabin pressure is greater than the air pressure in your ears so you would need to either, yawn, swallow or valsalva (forcibly breath out while hold your nose and mouth closed) to equalize the pressure, which I would think these would interfere with.

Q: Do these keep all the water out? I have tubes through my are drum so I can't get water in them.

I have tried lots of ear plugs which haven't worked at all. These are the first that I have been successful with that have kept water out. The one issue I have had is inserting them to get a consistent fit. Sometimes, one of the plugs will fall out while I am swimming. When that happens, it is easy to notice and find due to the design. I have learned to take extra time in inserting the plugs to get a snug fit and that has helped.
