How am I supposed to wear these?

That´s what we call an open question ! I wear them with golf socks, and as the toe area is wide they are very comfortable that way. The best of these shoes is that you can wear them on and off the bike.

Do these shoes work well with flat pedals as well as spds, or is that unrealistic? thanks!

It works perfect with both pedals. My pedals are double face, one flat and the other with clip and i ride perfect on both.

Will these shoes work good with standard pedals without the cleats?

Yes. I do not have the cleats in and use them with my traditional pedals. I do have toe cages, but I do not have to use those with these shoes. They're also pretty comfortable to walk in.
