Where is the size 12? this product appears to skip 12 and go from size 10 to 1/4?

#12 is not 1/4"---imperial machine screw sizes are in 64th's--- Therefore, #12 = 12/64 = 6/32 = 3/16---NOT 1/4

I thought it supposed to have thread in it

This is a thread gauge. The thread gauge is on the back side of the gauge. One of the listing pictures clearly shows this.

How does this tool define internal thread pitch (e.g. nuts)?

This gauge doesn't measure "internal" threads, only external threads. It can find the size of a nut but not the thread pitch of a nut. However, if you know the size of the nut then it's pretty easy to determine the thread pitch. There's only two thread options per size so just look at the threads and you can see if it's the coarse thread or fine thread. For instance, if the gauge determines that the nut is 1/2" then your two thread options are 13 or 20. The gauge shows you that part. If the threads look fine then you have a 20 thread if it looks coarse then you have a 13 thread. Hope this helps.
