how often should the nasal pillow be changed out?

I number my nasal pillows and rotate them on the first of the month, I believe they are good for 2 to 3 rotations. I buy 5 extra nasal pillows when I purchase a complete cpap DreamWear mask, then I am stocked for a year.

I have 4 pillows that I rotate monthly.....so each pillow gives me at least 3 months of usage if not more......If the pillow leaks too much, it's time to replace.

do i need the frame also to were this one ?

Yes you do. The air travels through the frame to the nose cushion. There is a hose connector on the top of the frame (keeps the hose away from your face) and an elastic belt to hold the frame on the head. This is an extremely comfortable setup and I highly recommend it to anyone using a CPAP. It is made of very "soft", thin silicone type plastic. Very easy to sleep on your side or back. Had a great night sleep on the first try. Does not irritate the nose/face like the typical CPAP masks. One note, there are specific measurements for the frame. Do some research online to get the correct size and complete package.

does this replacement cushion have the nasal pillows that fit into the nose or just fit over the nose?

It fits under the nose. I tried the nasal pillow cushions that fit into the nostrils and simply couldn’t use them. They were so snug, and I felt like I couldn’t breathe at all.
