Which side is in or out, or does it matter?

The side with the clear plastic liner is the inside, You have to remove that as you wrap it onto your handle. It doesn't stick though, so you still need to wrap tape around the the bottom.

How thick is this grip? I'm wondering if I should downsize my racquet by 1/8"

It's about the same as most over-grips. I don't think many people downsize for an overgrip - it's probably a half-grip size. Try it, and if it's too thick, have a racquet shop shave your grip down a tiny bit.
